Acupuncture Applications
The Professional Liability Insurance Application for Claims-made Coverage for
Acupuncturists is available in two formats as follows:
- PDF - This version of the application can
be downloaded for printing if you have Adobe Acrobat reader on your
computer. Print the application, fill it out, assemble all supporting
material, and mail it to the address on the form. If you need
Acrobat reader click on the icon below to download it.
- HTML - This version of the application can be
filled out online in an interactive web form. You will still have to
print the resulting document and mail it along with supporting material, but
it will print pre-filled with the responses you supplied online. It will
also specify exactly what supporting materials you need to supply based on
your responses.
